Special Olympics Athlete Medical Form - Required for Participation
Important Update (effective 6/30/2025): SOMD has now transitioned to the use of the Athlete Registration form. This form is required annually and removes the need for a medical professional's signature for virtually all Special Olympics Maryland's athletes.
Important features of the new form:

Physician signature NOT required for sports participation and competition within Maryland

Physician signature required for events outside Maryland (e.g., national competitions)

Medical questions are completed by parent/guardian/caregiver

Waivers combined; only 1 signature needed

Typed signatures are not accepted; electronic signatures such as DocuSign are accepted

All old medical forms will expire on 12/31/2025

The old medical form should no longer be used and will not be accepted after 6/30/2025

All athletes must have the new form starting with the Winter 2026 sports season

U.S. Athlete Registration Form is available on the SOMDPGC website

New forms should be submitted to pgcspecialolympics@gmail.com by 12/31/2025

Thereafter, the U.S. Athlete Registration Form must be updated annually
